Some insurance companies require referrals to make an appointment with a specialist. We require 48 hours for a referral to be processed.
Every effort should be made by you to make your physician aware of any refills that you may need at the time of your visit. If a visit is not scheduled and you need medications, we would prefer that you call your pharmacy to make the request. They will contact us directly. We require a 48 hour notice on refills. Calling to check on your refill request in less than 48 hours may delay the process.
Forms left to be completed by a provider will be completed in seven business days, unless there are unusual circumstances. There is a $15.00 charge per form, which is due prior to completion.
We file insurance claims as a courtesy to our patients. This includes all major insurance carriers with the exception of automobile insurance carriers. Co-payments are a requirement of your insurance company and must be paid at the time of your visit. Co-payments cannot be waived for any reason. Balances not covered by your carrier are your responsibility and payment is required within 30 days. Physicals such as adoptions, camp, marriage, sports and for CDL licenses are usually not covered by your insurance carrier. Check your benefits. Otherwise, payments for these types of physicals are required at the time of service.
Because our appointments fill up quickly, we request that you give our office a 24 hour notice of any cancellations or if you need to reschedule your appointment. Patients who do not show up for their appointment or fail to cancel their appointments within 24 hours will be charged a $40.00 cancellation fee.
